Dear Mr. customer:                         

Your sister expired earlier this month after undergoing surgery to remove polyps from her colon. You are unsatisfied with the explanation given by the physician and want to know if your sister received appropriate care given her previous medical history. You stated that Dr. doctor name, a gastroenterologist, was doing the colonoscopy and you now request a similarly qualified specialist. The expert, Dr. doctor, board certified in gastroenterologist (or alternate if there is a conflict of interest) will evaluate copies of the original records in order to determine if there is negligence and/or deviation from standard care. On completion of his review, he will provide a written statement. The actual fee ranges between $1,500.00 to $2,000.00 (based on chart size and specialty) paid to Health Information Search, Inc. This fee covers our location fee, the doctor's fee for his time to review and write a narrative. We neither warrant nor make any representation as to any reviewing physician's opinions. Specifically, we do not agree to provide a medical opinion that supports your ideation of the case. Instead, we agree to provide a doctor who will render his opinion based on his education, training, and experience. We will make every effort to provide an opinion as soon as possible if that is your request, but make no promise to provide an opinion by a date certain. It is further understood that this expert will be available for further work only through Health Information Search Inc.
